Reconstruction of the posterior mandible with vertical atrophy is up to date one of the oral surgeon’s greatest challenges. The inlay technique is one of the therapies of choice for the treatment of this increasingly common condition, with stable long-term results as attested by the literature. The surgical approach in question involves an interproximal bone graft, maintaining the patient’s native bone at crestal level, thus at the most critical point for future implants. This peculiarity, together with the vascularisation of the block from both the lingual periosteum and the surrounding bone surfaces, makes it possible to obtain biologically active bone.
